PerforMED® to Present on Care Management Approaches
for Complex Populations
Presentation to be made at California Association of Health Plan's
Patient Management and Mitigating Chronic Disease Seminar
Philadelphia, PA – Lynne Harsha, Associate Vice President, PerforMED®, the care management model from the AmeriHealth Mercy Family of Companies, will present at the California Association of Health Plan's (CAHP) Patient Management and Mitigating Chronic Illness Seminar on Wednesday, February 24.
Ms. Harsha will present Design and Development of Customized Care Management Approaches for Complex Populations at 1:00 p.m. that day. She will discuss the case study of the development of a Medicaid focused delivery model that addresses the aged, blind and disabled (ABD) population, including clinical and financial goals and outcomes.
The CAHP seminar, taking place at the Hilton San Francisco Airport in Burlingame, will discuss tools that health plans can use to help control chronic disease and techniques to control costs, including, for example, transitional support, customized care management and congestive heart failure initiatives.
